Transplant Social Workers

Social Work is a regulated profession that puts people and their well-being at the centre. Social workers support you to reach your goals and improve your health and social well-being. We do this by offering advice, advocacy, safeguarding, and helping you feel confident and in control.

We have an open-door policy and are very happy to see people at any time about any issue causing concern. We will always try and help or signpost to the most appropriate service. Appointments can also be made.

What We Do

Our day‑to‑day work focuses on understanding what you and your family need. We look at your strengths, what matters to you, and any goals you want to work towards. We also share helpful information about health, well-being, and behaviours that may affect your health.

 

Making Changes and Getting Support

We help you and your family make positive changes, solve problems, and take part in activities that feel meaningful to you. We also connect you with other services when needed, to help improve your quality of life.

Some of the areas we support include:

 

Transition to Adulthood

  • Helping young people and families prepare for taking more responsibility for their own healthcare.
  • Supporting the move from children’s services to adult services.
  • Making sure you feel confident and informed throughout this change.

 

Education and Employment

  • Guiding you to services that can help you access education, training, or work.
  • Making sure you know your rights, including reasonable adjustments.
  • Providing health statements to support your education or employment goals.

 

Advocacy and Signposting

  • Making sure your needs are heard within the healthcare team.
  • Linking hospital services with community support.
  • Helping you understand who can support you and how to access them.

 

Money Matters

  • Giving information and support around financial help such as housing costs, disability‑related benefits, welfare schemes, and grants.
  • Signposting you to services that can help with budgeting or debt advice.

 

Support at Home

  • Offering guidance for anyone finding it difficult to manage daily tasks at home.
  • Helping you find practical support if needed.

 

Housing Support

  • Assisting you in finding safe, suitable accommodation.
  • Helping with issues linked to poor housing that may be affecting your health.

 

Transport Support

  • Helping with Blue Badge applications.
  • Offering advice on the Motability scheme, discounted travel, and hospital travel expenses.

 

Identity and Diversity

  • Supporting you with understanding who you are, your values, and how you see the world.
  • Providing a safe and respectful space to talk about identity.

 

Relationship Support

  • Helping you and those close to you during times of change or when difficult decisions need to be made.

 

Support for Carers

  • Recognising that being a carer can be challenging and affect everyday life.
  • Signposting carers to support, services, and practical help.

 

Bereavement and End‑of‑Life Support

  • Supporting individuals and families with emotional and practical concerns during serious illness or after a loss.
  • Ensuring that advance care planning is offered.
  • Helping you access any financial support you may be entitled to.

Waiting for a heart or lung transplant can feel overwhelming, and you don’t have to go through it alone. Our virtual pre‑transplant support group meets once a month on Microsoft Teams, usually on the last Friday of the month from 10–11am.

Each meeting includes a speaker who shares helpful information about life on the waiting list and what to expect after transplant. You’ll also have time to talk with others who understand what you’re going through.
